Insurance Claim Form
The Forensic Science Laboratory (FSL), Maharashtra, provides a facility for citizens to download and submit the Insurance Claim Form in cases where forensic reports are required for claim settlement.
Purpose
This form is intended for applicants (such as family members or legal representatives) to request FSL support in insurance-related cases where post-mortem or viscera reports are involved.
Instructions
1. Download the Insurance Claim Form from the link below.
2. Fill in all required details, including:
○ Applicant Name
○ Name of the Deceased
○ Relationship with the Deceased
○ Date of Death
○ Name of the Institute where Post-Mortem was conducted
○ Forwarding Letter Number & Date under which viscera was sent to FSL
○ Reason for Giving Priority
3. Ensure the form is signed and dated by the applicant.
4. Submit the filled form with relevant document via email to: dir.fsl@maharashtra.gov.in
